Make sure you kiss River both instances when given a chance and to confirm your romance with River and lock it in for the rest of the game, be sure to select the "I feel good around you" option when that pops up. You must select the dialogue options that don't have you turning down River, which are pretty obvious (it'll be as obvious as "Kiss River" vs "Don't" and to continue the romance, you should kiss River). If you meed the above conditions, the romance encounter will begin in a scene that takes place atop the water tower in the side job, " Following the River," which is only unlocked by completing all three of the above "Romance River" options.
